Community Impact Letter of Intent

Purpose:

The Letter of Intent will be reviewed by the Community Impact Teams and evaluated on how the program addresses one or more of the focus areas and strategies, as they are developed and refined.  

 

The mission of United Way of North Central Iowa is

     Education: Helping children and youth achieve their potential.

     Income: Promoting financial stability and independence.

     Health: Improving people's health.

  • Expanding understanding of health issues

  • Achieving community results

Services area:

Requests for funding of services must demonstrate measurable impact to residents of the counties served by United Way of North Central Iowa. UWNCI serves: Butler, Cerro Gordo, Floyd (excluding Charles City), Franklin, Hancock, Kossuth, Mitchell, Winnebago, and Worth counties.

 

Funding Source:

United Way of North Central Iowa conducts an annual resource development effort for our regional community. Applicants must agree to cooperate fully in the annual fundraising effort and to encourage and enlist the participation of its constituency and members in such activities. The applicant will conduct an annual United Way workplace campaign amongst its employees and will solicit its' Board of Directors (where applicable) without the encouragement of designations to any particular program or partner.

 

Period of Performance and Payment:

The funding will be for a one-year term beginning April 1 and ending March 31 of the following year. All programs must submit the required documentation in the established format and by deadlines. Failure to do so will result in funds being withheld or forfeited. Reporting results is required on a quarterly basis, unless otherwise specified. A reporting form will be given upon award.  Performance will be measured using an outcome logic model. 

Eligibility Requirements:

Applicants are required to submit the following documents by January 15, 2011.

  1. Have an audit of the financial statements (balance sheet, statement of activities and statment of cash flows) by an independent certified public accountant whose examination complies with generally accepted auditing standards. Organizations with annual revenues totaling less than $200,000 and less than $500,000 in assets may submit compiled or reviewed financial statements prepared in accordance with the Statement on Standards for Accounting and Review Services issued by the American Institute of Certified Public Accountants. A copy of the most recently issued accountant's report and financial statements must be submitted by January.

  2. The organization's Board of Directors listing complete with their name, employer, position, board term, address and email.

Final Applications:

If notified to complete full application for funding: The original signed, by all partners and board representatives, application must be submitted to United Way of North Central Iowa.

Letter of Intent forms received after the due date and time (September 13, 2010 at 4:30 PM) will be rejected and not reviewed by the Community Impact Teams. Importantly, the request for and review of applications does not imply that funding will be provided to any organization or program. This depends on the volunteers, the prioritized community needs and the Resource Development results.
