Give Kids A Smile School Information

This year, each school will be asked to fill out a school contract. This will help United Way of North Central Iowa obtain more information on our schools and how they would like to partner with us for the Give Kids A Smile Program.

The school contract must be filled out and mailed or faxed to United Way prior to the dental screening date selected for each school.

There are 3 forms required for all participants in the Give Kids A Smile Program. We cannot see the kids if we do not have their forms. Please make sure each participant gets all three forms and they are filled out completely when returned.
